They key here is "not forwarded to the internet"

Without those forwards, your NAS will not fall victim to attacks via exposed ports.

In terms of malware remover .. you can only disable it (unless you modify your qpkg settings) .. Just disable it and that's all.

Malware Remover can only do what it has been designed for.
It can: Search and remove malwares that have been identified by QNAP, and when running a scan job.
It cannot: Search for unknown or new malwares, or in real-time.
So any malware can infect the NAS between scan jobs.
In short, it is useful to remove known malwares, not much to "protect" (to prevent an infection).

In addition:
afaik MR only scans QTS system files, but no user data! So MR is not an anti-virus protection as McAfee or ClamScan!
