I have two Qnap NAS' - one in spain and one in the uk. Can i get them to synchronise over the internet?

Yup, just do a site2site VPN with capable routers and it's no problem
(make sure both LAN subnets are in different segments e.g. 192.168.10.0/24 and 192.168.20.0/24)

Both are in Private homes - vpn not easy .. as neither site is guaranteed a fixed IP - i didn't know if there was a way through qnap cloud maybe?
Or even a manual link up as needed?

No fixed IP can be circumvented with DDNS
There is solutions with 3rd party VPN service hosting like open VPN access server (could be any other VPS you have access to as well)
